Job Requirements The Clinical Documentation Specialist is responsible for providing leadership and direction to ensure, through a concurrent review program with education, that the organization's documentation practices are appropriate. The Clinical Documentation Specialist will provide guidance to the clinicians and non-clinicians who will interact regularly with physicians, management, and case management staff in a way, which ensures clinical documentation practices, are consistent, accurate and efficient.ESSENTIAL ACCOUNTABILITIES: Conduct concurrent documentation reviews for identified patients in the required timeframe. Communicates with physicians face to face or via clinical documentation inquiry forms to clarify information, obtain needed documentation and educate for appropriate MS-DRG base on severity of illness. Follows up with appropriate health team members to ensure accurate and complete documentation in the medical record. Demonstrates an understanding of complications, comorbidities, severity of illness, risk or mortality, case mix, secondary diagnosis, impact on procedures on MS-DRGâs and is able to impart this knowledge to physicians, and other health team members. Confers with Physician Advisor as needed. Develops and conducts education programs on a regular basis designed to improve the accuracy and consistency of documentation by physicians and clinicians. Supports the efforts of case management staff where this participation and communication model is used. Conducts documentation audits at least on a quarterly basis. Develops and maintains a frequently asked questions database Works with HIM and Care Management to periodically analyze coding data of the hospital to identify documentation variations and determine the cause and the appropriateness of such variation and presents such findings to the HIM Director. Ensures that the facility clinicians document in a manner consistent with relevant laws, regulations and standards. Prepares annual work plan related to clinical documentation improvement planMINIMUM EDUCATION, CERTIFICATION AND EXPERIENCE1. RHIT, RHIA, CCS, RN with inpatient coding experience.2. At least four (4) years experience in inpatient coding, in particular Medicare, with some supervisory experience in a hospital and/or clinic setting. Demonstrated ability to develop and conduct education programs for physicians, Case Managers, other clinicians and HIM/Coders. Experience in conducting efficient and regular documentation reviews and evaluating the quality and efficiency of documentation practices is also required.ESSENTIAL KNOWLEDGE, SKILLS AND ABILITIES1. Excellent written and verbal communication skills, including the ability to present ideas and concepts effectively to Physicians, management, and employees (clinicians). Strong knowledge of DRG methodology and severity of illness/risk of mortality statistics.2. Thoroughly familiar with coding clinic guidelines, knowledge of disease process, knowledge of anatomy and Physiology, and the ability to explain to the physicians and Clinicians what is required when the documentation is lacking.
